  • When it’s hard, start with the round ligament
  • Be lateral enough to be in good tissue
  • Dissect down to the cuff
  • Deflect posterior, skeletonization of the uterine vessel
  • Take tissue below bladder flap
  • Don’t be afraid to switch out instrumentation for safety
  • Trick: You can backfill the bladder
  • Goal: Obliterate enough until you have good skeletonization and it looks like a normal hysterectomy surgery
